How I do apply for visiting Taiwan for seeing the sick, or extending the stay for caring the sick?

Who should file the application?

The application must be filed by:

I.The relative being visited

II.Relatives or friends living in ROC

III.Travel agent

Suitable Candidates

I.People of Mainland China with 3rd degree relatives, spouse? parents, spouse or spouses of children with ROC household registration, may apply to enter ROC for visiting purpose if one of the above parties have one of the following situation:
1.Life is in danger due to disease or injury
2.Age over 60, with disease or injury

II.Parents or children of Mainland China spouses of ROC citizens residing in ROC in accordance with the principle of dependence or as permanent resident may apply to visit ROC if the spouse of the ROC citizen is sick or injured and life is in danger.

III.Applicants of the previous 2 clause who are above 60 years old, diseased, or disabled may apply for a 2nd degree relative or spouse to accompany him/her into ROC.

Where to apply?

I.Applicants who are residing in other countries should file the application to a ROC embassy, representative office or organization authorized by the Ministry of Foreign Affairs located in the country.

II.Applicants who are residing Hong Kong or Macau should file the application to the Taipei Economic and Cultural Office in Hong Kong, address: 40th Fl., Tower One, Lippo Centre, 89 Queensway, Hong Kong; or Taipei Economic and Cultural Office in Macau, address: Al. Dr. Carlos d'Assumpcao, No. 411- 417, Edif. Dynasty Plaza, 5 Andar J-O, Macau.

III.For applicants residing in China, the application must be filed by a relative staying in ROC to the National Immigration Agency service centers located in various cities or counties. Postal application will not be accepted.

Required documents

I.Please fill in the application form for people of Mainland China entering ROC with all the details required. Please attach a 4.5x3.5cm in full color ID photo taken within 2 years. Hat must not be worn in the photo, and the party must not wear colored glasses, the facial features must be distinguishable and the length of the party? head must not be smaller than 3.2cm or larger than 3.6cm. The background of the photo must be white in color and must not be edited.

II.A copy of the Mainland China identification certificate or other documents certifying the applicant? identity.

III.Certified documents proving applicant? relationship with the visited party. For those have provided the aforementioned documents during previous applications, please state in the application form that the applicant has been to ROC previously and countersign on the statement. These documents can then be omitted after the inspecting authority verifies that the documents were previous provided.

IV.Household registration or ID card of the party being visited. Please provide both copy and original. The original will be returned after verification.

V.A letter of indemnity: please provide a letter of the indemnity signed by the guarantor, original and copy of the guarantor? ID card. The original copy will be returned after verification by the National Immigration Agency centers of various cities of counties. Please refer to the descriptions on the back of the letter of indemnity for qualifications of the guarantor.

VI.Relative documents: Catastrophic Illness certification issued by the National Health Insurance Bureau or a diagnosis certificate issued within 3 months by a private or public hospital authorized by the Department of Health, Executive Yuan.

VII.Authorization letter: Not required if the applicant is staying in a foreign country, Hong Kong or Macau and the application is filed by the visited party personally. An authorization letter must be provided if the application is filed by other relatives or friends in ROC or by a travel agent.

VIII.If the applicant is staying in a third country, a copy of the third country? entry permit, residence permit, or Hong Kong ID card must be provided.

Fees

The fees of the permit is NT$600. No additional cost will incur if registered return mail envelope is provided or if the permit is collected personally.

Fees for extension is NT$300. The application must be filed to the National Immigration Agency service centers located in various cities or counties.

Things to know for application

Extension method: the applicant must provide all the necessary documents for application 30 days prior to the expiry date.

III.Household registration 2 within 2 months with all the residents registered.

IV.Deposition stating that the applicant has no children in ROC

V.A diagnosis certificate issued within 3 months or a valid Catastrophic Illness certification issued by the National Health Insurance Bureau.

VI.A valid Mainland China entry/exit pass or passport. Extension will only be granted to 1 month before the permit or passport valid date

VII.A colored photograph of the specified dimension taken within 1 year.

VIII.Fees for extension is NT$300. The application must be filed to the National Immigration Agency service centers located in various cities or counties.
