Hong Kong and Macau Residents

I am under the age of 12 and I have an entry/exit permit. My parent has household registry in ROC, how do I apply for permanent residence?

一、 Laws and regulations:Policies for Entry and residence permit of people of Hong Kong or Macau who wish to enter ROC.
二、 Suitable candidate:People of Hong Kong or Macau under the age of 12 with a valid entry permit and the parents are residing in ROC with household registry.
三、 Required documents:
(一) An application form for people of Mainland China entering ROC with all the details filled. Please attach a 4.5x3.5cm in full color ID photo taken within 2 years without hat (same dimension as the ID photo).
(二) Original and copy of the Permanent ID of Hong Kong or Macau. The original copy will be returned after verification.
(三) Health examination certificate (A health examination report as per the requirements of the Department of Health. The report must be issued within 3 months). The health report is not required if the party is below the age of 6 but a copy of the vaccination report must be provided.
(四) Other relative certificates:
1. The original and copy of the birth certificate authenticated by Taipei Economic and Cultural Office in Hong Kong or Macau, or a ROC embassy. The original will be returned after verification.
2. Entry/exit permit.
3. The original or copy of the parents’ marriage household registry or National ID. The original will be returned after verification.
4. Underage applicant with mother or father residing in ROC alone must provide a consent form signed by both parents (A consent form issued by the guardian or the party with the child’s custody must be provided if the parents are divorced).
5. Child born out of wedlock must first be adopted and the DNA report and prove of status during pregnancy must be provided. If the pregnancy occur before the marriage, the DNA report and prove of status during pregnancy must be authenticated by the by Taipei Economic and Cultural Office in Hong Kong or Macau, or a ROC embassy. (The gestation period may be calculated as day 181~302 prior to the date of birth).
(五) Authorization letter: An authorization letter must be provided if a representative is appointed and the letter must be signed or stamped by the representative personally. The application form must be signed or stamped by a representative of the travel agent if a travel agent is employed for the delivery of the application. An authorization letter is not required if a travel agent is employed.。
(六) Permit fee: NT$600
(七) Please provide a registered return mail envelope with the recipient’s name and address, postal code, and telephone number.
(八) Authentication certificate for Hong Kong or Macau Identity.
四、 Attention:
(一) Certificates issued in Mainland China, Hong Kong, Macau, or other countries must be authenticated by Straits Exchange Foundation, a ROC embassy, or the Taipei Economic and Cultural Office.
(二) According to the policy for residents of Hong Kong or Macau entering and residing in ROC article 5, clause 1 and 2: application to enter or reside in ROC with missing documents must be provided within 3 days after receiving the notification. The application will be denied if the applicant fails to do so or the provided documents do not meet the requirements.
(三) The age calculation for underage applicants below 12 years old: according to the actual date of application at the service center.
